MILWAUKEE — Researchers at Versiti Blood Research Institute have identified a previously unrecognized class of antibodies involved in heparin-induced thrombocytopenia (HIT), a serious immune complication that can develop after treatment with the blood thinner heparin. Published in the journal Blood, the findings challenge current understanding of how HIT develops and suggest that existing diagnostic tests may miss some clinically important cases.

HIT is a rare but potentially life-threatening condition in which the body's immune system produces antibodies that activate platelets, increasing the risk of dangerous blood clots while causing platelet counts to fall. Because prompt diagnosis is critical, clinicians around the world rely on laboratory testing to determine whether patients have the condition.

The study found that some disease-causing antibodies are not detected by the standard enzyme-linked immunosorbent assay (ELISA), a test widely used to rule out HIT. Despite escaping detection, these antibodies strongly activated platelets, suggesting they may play a central role in driving the disease.

Improved testing could help clinicians diagnose HIT earlier and more accurately, reducing the risk of life-threatening clotting complications while guiding safer, more effective treatment decisions.

The research also expands scientific understanding of platelet factor 4 (PF4), a protein involved in immune-mediated clotting disorders. Recent landmark studies have demonstrated that "rogue" anti-PF4 antibodies can cause vaccine-induced immune thrombocytopenia and thrombosis (VITT). This new work extends that evolving understanding to HIT, suggesting that PF4-mediated immune responses are broader and more complex than previously appreciated.

The study positions Versiti Blood Research Institute at the forefront of research into immune-mediated thrombotic diseases by uncovering mechanisms that could inform future diagnostic assays and precision therapies.

The publication, "PF4/Heparin-Nonbinding Platelet-Activating Antibodies in Heparin-Induced Thrombocytopenia," appears in Blood.

The research team included lead author Lu Zhou, PhD; Andrew Cao, an MSTP student; former doctoral student Wen Zhu, PhD; and senior investigators Brian Curtis, PhD; Richard Aster, MD; Demin Wang, MD, PhD; and Renren Wen, PhD.

Although HIT is uncommon, it is a medical emergency when it occurs. Accurate diagnosis is essential because patients require immediate changes to their treatment to reduce the risk of potentially fatal blood clots. By identifying antibodies that current screening methods may miss, this research lays the foundation for more sensitive diagnostic tests that could improve patient safety and care worldwide.
